    Must be read, signed, dated and returned. Please note that a LAB FEE of $15.00 is required for CPR/FIRST AID Certification. Please make checks to Liberty High School and submit to the school banker for further processing. Cash is also accepted, but must be processed accordingly. Banker schedule are in the morning, after school and during your lunch. Deadline for payment is scheduled for December 13, 2012.
